JOB POSTING
CASE MANAGER
The successful candidate will provide case management to people with severe mental health problems/serious mental illness who are mainly men from the Caribbean community. There will be a caseload of 15-25 and the case manager's contact with clients will take place in the community and at times in the office. The case manager must demonstrate a full understanding of anti-racism, awareness of mental health issues of individuals and groups of colour - particularly as it relates to men from the Caribbean community. Lived experience of racism and xenophobia and an understanding of their impact on health and mental health is a must.
Responsibilities:
- Proactive outreach in the Caribbean community and client identification
- Conducting risk assessments that reflect a comprehensive holistic approach, including assessing situations of high risk e.g. suicide, violence, psychiatric and medical emergencies
- Direct service provision (counselling, skills and health teaching, crisis intervention)
- Coordination of services and client advocacy
- Monitoring and evaluation of client's condition
- Program Support
- Record keeping
Experience and Skills:
- Excellent communication and interpersonal skills
- Knowledge of case management functions
- Understanding of the concepts of client-centred relationship
- Good English language skills (written and verbal)
- Degree or diploma in social services or related field
- At least two years experience in human services, preferably as a case manager
- Strong understanding and commitment to anti-racism/ anti-oppression
- Knowledge of Internet and Microsoft Word
- Lived experience/in-depth knowledge of the Caribbean community
- Ability to work independently as well as with a team
- Second language an asset
